Teacher Materials Request

The Algonquin Area Public Library is pleased to offer you the service of pulling materials that may be useful to you or your students as you study a particular unit in your classroom. Materials checked out are subject to the same lending limits and fines as other library materials. Patrons are limited to 15 items per request. Please fill out the following form if you’d like to have materials pulled for checkout.

If you would like to arrange a class visit, library tour, or storytime please contact the library at 847-458-6060 and ask for the Youth Services desk.

Briefly describe the unit you are studying or the assignment your students need to complete.

The library requires at least 3 week days notice to fill a materials request.
If you have a library card from a public library, please enter the barcode number.
Materials will be pulled by the date requested and held at the location requested for 5 days. Materials not picked up after 5 days will be returned to the shelves. If you have any additional questions, please call the Youth Services Desk at (847) 458-6060 x135.
